Appointments
Appointments are an important part of your
child's treatment. This time is reserved especially
for you and your child with the doctor.
Please arrive on time for your appointment.
If you are more than 10 minutes late, you may
be asked to reschedule.
We request at least 48 hours notice if you need
to cancel or reschedule.
Missed appointments may result in dismissal
from the office.
Insurance
Our office is committed to helping you maximize
your insurance benefits.
We will gladly verify your benefits and help you
understand your coverage.
We will submit claims to your insurance company
as a courtesy.
Estimated patient portions are due at the time
of service.
Coverage varies by plan. Your estimated portion
may change based on your insurance contract.
Payment Options

dental care affordable for your family.

Payment is due at the time of treatment.
We will review all costs before treatment
